I have had bags under my eyes since I was in my 20′s and I am frequently asked by friends if I am tired. I feel fine! I just want to look as good as I feel. Can these be removed, and if so, how?

A:

“Bags” under the eyes are caused by the protrusion of fat from around the eyeball. This displaces the skin and muscle of the eyelid and creates the appearance of bags or circles under the eye.

Blepharoplasty is the medical term for eyelid surgery which improves the appearance and/or function of the eyelid. In patients who are relatively young and don’t have much loose skin, the fat can be removed without having to make an incision through the skin. This procedure, called transconjunctival blepharoplasty, avoids an external scar and shortens the recovery period.
For those with loose skin, the incision is made just beneath the lower lash margin, fading completely within 3-6 months.

Mineral Makeup is a popular alternative as the population has grown conscious of what we are putting onto our skin and bodies. This awareness, has people asking more questions. Recently, attention has been focused on a common ingredient found in mineral makeup known as Bismuth Oxychloride.

Bismuth is the by-product of lead and copper refining. Companies use this in their cosmetics because it is cheap to buy and is an inexpensive filler. It does have binding qualities, so the makeup will “stick” to your skin. Its molecular make up is often viewed as pearlescent or shiny . This makes it highly refractive, which is something companies want. Refraction will camouflage fine lines, wrinkles and discoloration. If you use a brand with Bismuth Oxychloride you’ll notice a shiny look after applying the makeup. Not like oiliness, but almost shimmery. That’s the Bismuth Oxychloride.

Its molecular makeup is a crystalline shape, which may cause itchiness for some people when they wear it or when they sweat. It has to be forced into the pores (hence the buffing technique) otherwise it is capable of sliding off the user’s face. It is also a heavy element, which makes it difficult for many people to wear. This heaviness can result in clogged pores, papules, pustules, whiteheads, extreme blackhead problems, itchiness, irritation and more.

Check your labels for Bismuth Oxycloride as it is found in many of the popular brands. Here at the Petroff Center we utilize two techniques for Hair Transplantation: Micro-Grafting and Mini-Grafting. Facelift surgery has evolved significantly over the years. Patients no longer have to accept some of the tell-tale signs of facelift surgery of the past such as altered hairlines, wide scars, or a “pulled look.”

With the advent of liposuction to remove unwanted fat in the neck and jowl region, we are able to sculpt these areas to achieve a more youthful jaw line and neck. This, in conjunction with correct placement of incisions to camouflage the scar and preserve the hairlines, creates a natural look.

For someone who specializes in facial plastic surgery and has performed thousands of facelifts throughout my career, there is no miracle cure for the signs of aging. I am routinely asked about “mini-lifts,” “limited incision lifts,” the “S-lift” and the list goes on. Typically, these modified facelifts are targeted to signs of aging in a particular area of the face. This procedure is appropriate for someone who may have already had a lift, but once again is showing some signs of aging or a younger person who is starting to exhibit signs of drooping jowls. These procedures give your surgeon access to limited areas of muscle, fat and excess skin. Hence, the results are often limited and do not hold up over time.

As we age, gravity starts to take its toll on the underlying tissues of the face and neck. The support structure of the face (muscle, skin and fat) begin to sag and other signs of aging become more pronounced. Any loosening or sagging of the muscle and skin will require surgery to treat. A deep-plane facelift actually lifts the underlying muscle and tissue, putting less tension on the skin. Excess skin is surgically removed through incisions that are carefully hidden along the hairline and within the anatomy of the ear.

A facelift can offer dramatic results. A well-trained surgeon with an artistic eye will aim for natural-looking results and will not recommend a modified or limited facelift in a candidate that has significant signs of laxity of the skin and underlying muscles.

Candidates should know their surgeon’s background and training. Check to make sure that the surgeon’s area of expertise and corresponding board certification is in facial plastic surgery.

Look at examples of before and after photographs. Verify that these photographs are examples of their work. Request photos that represent patients of your similar age.

Request references for patients that have had the same procedure your physician is recommending. Are these patients happy with their results?

FDA MedWatch Consumer Warning

The FDA notified healthcare professionals and is warning consumers not to use skin creams, beauty and antiseptic soaps, or lotions that may contain mercury. The products are marketed as skin lighteners and anti-aging treatments that remove age spots, freckles, wrinkles, blemishes and acne. Products with this toxic metal have been found in at least seven states.

The products are manufactured abroad and sold illegally in the United States–often in shops located in Latino, Asian, African or Middle Eastern neighborhoods as well as online. Consumers may have also purchased them in another country and brought them back to the U.S. for personal use. The FDA has identified 35 products with unacceptable levels of mercury in the past few years.

Consumers are advised to check the label of any skin lightening, anti-aging or other skin product used. If there is no label or no ingredients are listed, do not use the product. If “mercurous chloride,” “calomel,” “mercuric,” “mercurio,” or “mercury,” are listed on the label, stop using the product immediately. If you suspect you have been using a product with mercury, stop using it immediately. Thoroughly wash your hands and any other parts of the body that have come in contact with the product. Contact a healthcare professional or a medical care clinic for further advice.
