After 15 successful years of practice at St. Vincent's Hospital, our new facility opened in August of 2003, conveniently located off of I-5 in the beautiful city of Lake Oswego. Click here for a map and directions.
It has always been our goal to provide the highest quality service. We have built an extraordinary facility that complies with state licensing and federal certification requirements for participation in the Medicare program. The Petroff Center is a State Licensed Type I Ambulatory Surgery Center designed to deliver the highest quality care in a safe and pleasing environment.

Patient Information
The following forms can be downloaded to your computer and printed at home. In order for Dr. Petroff and Dr. Maier to provide a complete and comprehensive medical opinion it is important that they review all your medical information. Please be prepared to provide this information to our office along with your current medical insurance information. Below is a link to our patient information forms. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.
